At the time of your first appointment in our office, you will be asked to complete a registration form which asks for personal information such as name, address, social security number, insurance information, and past health history.  You will also be asked for a photo ID.  Please bring this information with you.  It is very important that we maintain accurate, up-to-date information about you in our files.

Please inform the receptionist if you have been referred to our practice by another physician so that we may be sure to send our physician’s evaluation and treatment recommendations to that physician.  We will also keep your referring physician informed of results of any tests we perform.  If you have had MRI, CT scans, or x-rays pertinent to your problem, please bring these films to your appointment, to assist in assessing your medical condition.  Please bring all current prescription and over-the-counter medications with you on each visit to our office.
